I grant permission to Lakeshore Academy of the Arts to take photographs, videos, or other media for use in marketing, advertising, social media, website, newsletters, magazines, recruiting brochures or emails.

I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is unknown to me or unknown, and I waive any right to royalties or other compensation.

I do NOT give consent to photographs or videos of my child.

I understand that it is my responsibility to Lakeshore Academy of the Arts LLC to make tuition payments by the first of each month for that month. Should I decide to withdraw from classes prior to the month's end, no refunds shall be given. A one month notice is required for withdrawal from dance classes. Accounts with a past due balance will result in termination from the Academy.
